Wideband and CEL

Ever since I installed my wideband I get a CEL code for the oxygen sensor every once in a while. I have it set so it is running in open loop and have the heater disabled. Also when ever the wideband voltage it above 2.50V the error LED in hondalogger lights up. Any ideas?

I get the same thing, I'll just be driving and I throw the O2A CEL code, and I have the O2 heater disabled and I'm running open loop as well, some one help us out?
