AFR values stuck at 11.5 range no matter what

Hello, I have a full k20a2 (04 to be exact) setup with full bolt-ons (skunk2 street manifold, 74mm tb, kpro v4, fic injectors). The car was dyno tuned and ran really well up until recently. The afr value used to read a high in the 13.0 range and dipped into the high 12.8 range. Now recently, I noticed the car going through gas like crazy (50 miles used up half a tank and this is highway mileage). The afr is always stuck at 11.5 some time 11.6 range at idle and WOT. I looked at the hondata app and noticed that the afrcmd is reading within the range of what the car used to run at: 14 for idle, and 12.8 when WOT). Car heats up like crazy also, I have to turn on the heat to cool down coolant temp. Also I noticed that no matter what I do to cool down the intake air temp, it always reads high, even on days with ambient temps of mid 60 degree. The temps would climb upto 122 degree for the intake air temps. The car intake air temp used to read in the 60 - 70 degree range while on the high way doing 65mph.

I have tried changing the o2 sensor, and this didn't change the afr reading. I tried three brand new ones and all the same result. I have also changed the throttle body gasket bc it was bad (fount this from bad vacuum messing with the map sensor reading. The map sensor is a 4bar. I tried another tps sensor, calibrated it to 0.446V closed throttle and 4.86V open throttle with a volt meter, and made adjustments to the throttle in Kmanager. Any help and guidance would be greatly appreciated.

Without the calibration and a datalog showing the issue, it's hard to say what it could be.

I haven't got a hold of the tuner yet. I'll go do a few data logs and ill post it with the calibration. Ill add them shortly

additional info: serial number is 47436. Software of kmanager is 4.4.4. Ordered kpro new from HPTautosport on March 13th 2018. Engine harness is a ktuned without charge harness (made one custom). Ktuned vss conversion adaptor. Flex fuel sensor installed.

What was the cause of this? My Kpro v2 is doing the exact same thing.
