Found the problem. VTEC spool error disable was checked in the software. When I unchecked it, I got a CEL and fixed the issue (bad splice). The car revs comfortably to 8K now and pulls all the way.

I also noticed that the cal was in open loop and that the stock 02 sensor threw a CEL so I've changed the cal to use the wideband feedback and run closed loop but I think I need to lower the target voltage as after driving it a few days the AFRs seem a bit lean.
