Ah crap... it is in a 2005 Acura RSX Type S. The motor WAS the original K20Z1, and was recently swapped to a DC5R K20A due to Hydrolocking the Z1.

I literally just did this exact swap. Willing to bet you swapped the VSS and Tach wiring as it isn't THAT hard to do lol

It appears to be related to the battery voltages as they are different in the different gears.
Interesting. I do see the voltage going from say 13.8 down to 13.2 in steps in 3rd-4th& 5th
Whats weird is at 13.3v in 3rd... no issue. Even dipping to 13.0v at 8500rpm no change in CMMD AFR

With how you and the database explains that DTC is triggered in the 1st place, that has to be it. Must be looking for something like that on initialization. Thanks for the help. I'll update with result/fix.

Voltage going into the ECU, (using a T pin and multimeter for reference)
Original/"no issue" .kal - 11.7v was lowest I saw

I have a fully tuned map for more or less that exact setup but its a PRB... AFAIK the data points will populate in the fuel/timing/etc maps but it won't work. There is a base map provided by HONDATA for very close to this exact setup.
Under PRB->RSX Type S->"k20a-itr"
This is a dyno tuned JDM DC5R K20A map for a car with the usual I/H/E. Depending on if your running an RBC/RRC, etc ypu may be able to squeeze a bit more top end or midrange out of it but ...
